Your Initial Appointment at Blue House Health

Generally Dr. Johnson spends two hours with you at the initial appointment.

If possible, please print the following forms and bring them, completed, with you to your appointment: Adult Intake Form, (or the Child Intake Form), Informed Consent Form, Insurance Reimbursement Form and Privacy Disclosure.

If you do not have access to a printer, please arrive 20 minutes early for your appointment to complete the paperwork in the office.
